With the growing number of long-term survivors of liver transplants, there needs to be more information and guidance on living life with a liver transplant, and on managing the conditions that impact the long-term health of the transplant recipient. GLI is bringing together Johns Hopkins and digital health catalyst Health 2.0 to co-design an innovation challenge to develop a decision-making tool includes risk-prediction analytics for co-morbidities and complications and that defines quality care measures to help patients and physicians make joint decisions around care, support and treatment.
